tinbox campaigns send
DISPATCH (send now). Two locks (PRD §7.2): the campaign must be approved (content lock) AND your token must hold the `newsletter:send` capability on a write scope (authority lock) — without it: a clear authz error, exit 1. Ledgered + idempotent: a retry re-enqueues only un-sent rows.
